I've been poking at this, too, in the context of finding out if/how MHRA pick up signals as part of its Pharamcovigilance. I'd add that a) the Ministry of Justice said "not us, guv" when I submitted an FOI about Regulation 28 reports and b) Death Certificates are collected by the General Register Office which falls under HM Passport Office !! I do get e-mails from people - I think that they subscribe to the newsletter thing so can contact me direct via that - not 100% sure as new to all this social media lark.

If that doesn't work post a reply and I'll use the e-mail you've put above.

As to the MHRA they are not picking up any safety signals and say in an answer to any questions about it "read the public assessment reports as that is what we used to say they are safe and effective".

Dude, seriously, they gave you the answer right there: "in the absence of a UK national death investigation service". It seems to me that the person who wrote that is heavily hinting to you that a UK National Death Investigation Service needs to be set up. Sounds like a good plan.

Write back and ask how a person might set up a UK National Death Investigation Service. See what they suggest. Then maybe we can follow their suggestion. And then your questions can at last be addressed.

I don't see the police, per se, on your list? I was thinking about making a report but didn't want to waste my precious time on people who clearly don't give a f*ck (I'm not dead and nor is anyone else I care about who listens to me). What do you think?

Individual Police Forces haven't done sh*t even if individuals within them do want to do something.

The college of Policing are the ones who issue guidance to Police forces in the UK and who can tell them what to investigate and how and so on.

Some of their guidance issued during the covid pandemic (haha) was interesting reading. Some actually outright stated that the legislation could not be enforced so don't bother.

National Police Chiefs Council - who the Chief Constables actually work for I understand which is why the Home Secretary cannot sack them.

I've been thinking for a while that getting these 2 organisations to start the ball rolling on investigations will get more done than trying to put in individual crime reports and so on.

Crimes against humanity go via the Office of the Prosecutor at the ICC:

I sent them an e-mail in Sept 2020 asking them how to go about starting an investigation and they said "gather lots of evidence then send it to us".

The reference number they gave me as my official notification they have my e-mail is:

The Hague, 5 October 2020

They said "As soon as a decision is reached, we will inform you, in writing, and provide you with reasons for this decision." but so far no further communication either way on a say, no or maybe decision.

Maybe they are waiting for a big pile of documents to land on their desk before they reply - wishful thinking?
